This Disclaimer, Declaration, and Undertaking is executed by the undersigned applicant in favor of Jamila Welfare Foundation, registered under the Societies Registration Act, XXI of 1860 bearing Registration No. KAR NO. 025 of 2025-26 and FBR Registration No. I051951 (hereinafter referred to as "JWF").
یہ دستبرداری، اقرار نامہ اور یقین دہانی درخواست گزار کی جانب سے جمیلہ ویلفیئر فاؤنڈیشن (رجسٹرڈ تحت سوسائٹیز رجسٹریشن ایکٹ 1860، رجسٹریشن نمبر KAR NO. 025 of 2025-26، ایف بی آر رجسٹریشن نمبر I051951 — بعد ازیں "جے ڈبلیو ایف" کہلائے گی) کے حق میں تحریر کی جاتی ہے۔
I, the undersigned, confirm that I am 18 years of age or above, of sound mind and reputable character, and voluntarily apply for membership of JWF. Upon acceptance of my membership, I hereby declare, acknowledge, and undertake as follows:
میں، مندرجہ ذیل دستخط کنندہ، تصدیق کرتا/کرتی ہوں کہ میری عمر 18 سال یا اس سے زائد ہے، میں نیک سیرت اور سلیم العقل ہوں، اور اپنی مرضی سے جے ڈبلیو ایف کی رکنیت کے لیے درخواست دیتا/دیتی ہوں۔ اپنی رکنیت کی منظوری پر میں مندرجہ ذیل اقرار، تسلیم اور یقین دہانی کرتا/کرتی ہوں:
1. I subscribe to the Memorandum of Foundation and shall uphold the vision, mission, objectives, constitution, bylaws, policies, and code of conduct of JWF at all times.
میں ہر وقت جے ڈبلیو ایف کے میمورنڈم، وژن، مشن، مقاصد، آئین، ذیلی قوانین، پالیسیوں اور ضابطہ اخلاق کی پاسداری کروں گا/گی۔
2. I acknowledge that my membership in JWF does not authorize me to represent, bind, commit, or act on behalf of JWF unless I have received prior written authorization from the Chairperson or any duly authorized representative of JWF.
میں تسلیم کرتا/کرتی ہوں کہ جے ڈبلیو ایف کی رکنیت مجھے جے ڈبلیو ایف کی نمائندگی کرنے، اس کی جانب سے پابند ہونے، وعدہ کرنے یا کسی بھی عمل کے لیے مجاز نہیں بناتی، سوائے اس کے کہ مجھے چیئرپرسن یا جے ڈبلیو ایف کے کسی مجاز نمائندے سے پیشگی تحریری اجازت حاصل ہو۔
3. I shall not misuse, misrepresent, or exploit the name, logo, letterhead, identity, goodwill, reputation, membership, authority, or any intellectual property of JWF for any personal, political, commercial, financial, religious, or unlawful purpose.
میں جے ڈبلیو ایف کے نام، لوگو، لیٹرہیڈ، شناخت، ساکھ، رکنیت، اختیار یا کسی بھی دانشورانہ املاک کو ذاتی، سیاسی، تجارتی، مالی، مذہبی یا غیر قانونی مقصد کے لیے غلط استعمال، غلط بیانی یا استحصال نہیں کروں گا/کروں گی۔
4. I shall not organize, announce, advertise, conduct, participate in, or collect funds through any donation drive, fundraising campaign, charity collection, relief activity, sponsorship campaign, crowdfunding initiative, or any other activity in the name of JWF without obtaining prior written approval from the authorized management of JWF.
میں جے ڈبلیو ایف کے مجاز انتظامیہ سے پیشگی تحریری منظوری کے بغیر جے ڈبلیو ایف کے نام پر کسی بھی عطیہ مہم، فنڈ ریزنگ کیمپین، خیراتی وصولی، امدادی سرگرمی، سپانسرشپ مہم، کراؤڈ فنڈنگ اقدام یا کسی دیگر سرگرمی کا اہتمام، اعلان، تشہیر، انعقاد، شرکت یا اس کے ذریعے رقوم جمع نہیں کروں گا/کروں گی۔
5. I shall not solicit, receive, or accept donations, cash, cheques, bank transfers, online payments, goods, services, or any other contribution on behalf of JWF without written authorization.
میں تحریری اجازت کے بغیر جے ڈبلیو ایف کی جانب سے عطیات، نقدی، چیک، بینک ٹرانسفر، آن لائن ادائیگیاں، اشیاء، خدمات یا کوئی بھی دیگر تعاون طلب، وصول یا قبول نہیں کروں گا/کروں گی۔
6. I understand and agree that any unauthorized use of JWF's name, identity, authority, or reputation, including unauthorized fundraising or donation collection, shall constitute a serious violation of the terms of my membership.
میں سمجھتا/سمجھتی ہوں اور اس بات پر متفق ہوں کہ جے ڈبلیو ایف کے نام، شناخت، اختیار یا ساکھ کا کوئی بھی غیر مجاز استعمال، بشمول غیر مجاز فنڈ ریزنگ یا عطیات کی وصولی، میری رکنیت کی شرائط کی سنگین خلاف ورزی تصور ہوگی۔
7. In the event of any breach of this Undertaking, JWF shall have the absolute and unconditional right to immediately suspend or revoke my membership without prior notice and may initiate any civil, criminal, or other legal proceedings available under the laws of the Islamic Republic of Pakistan.
اس یقین دہانی کی کسی بھی خلاف ورزی کی صورت میں، جے ڈبلیو ایف کو بغیر کسی پیشگی نوٹس کے فوری طور پر میری رکنیت معطل یا منسوخ کرنے کا مکمل اور غیر مشروط حق حاصل ہوگا، اور وہ اسلامی جمہوریہ پاکستان کے قوانین کے تحت دستیاب کسی بھی دیوانی، فوجداری یا دیگر قانونی کارروائی کا آغاز کر سکتا ہے۔
8. I acknowledge that I shall be personally, solely, and fully responsible for all actions undertaken by me without written authorization from JWF. JWF, its Trustees, Board Members, Office Bearers, Employees, Volunteers, Members, and Representatives shall bear no responsibility or liability whatsoever for any unauthorized act committed by me.
میں تسلیم کرتا/کرتی ہوں کہ جے ڈبلیو ایف کی تحریری اجازت کے بغیر میرے کیے گئے تمام اقدامات کا میں ذاتی، واحد اور مکمل طور پر ذمہ دار ہوں گا/ہوں گی۔ جے ڈبلیو ایف، اس کے ٹرسٹیز، بورڈ ممبران، عہدیداران، ملازمین، رضاکار، اراکین اور نمائندے میرے کسی بھی غیر مجاز فعل کے لیے کسی بھی قسم کی ذمہ داری یا جوابدہی نہیں اٹھائیں گے۔
9. I further agree to indemnify, defend, and hold harmless JWF, its Trustees, Board Members, Office Bearers, Employees, Volunteers, and Representatives from and against any claims, losses, liabilities, damages, penalties, legal proceedings, costs, or expenses arising directly or indirectly from my unauthorized acts or omissions.
میں مزید اس بات پر متفق ہوں کہ میں جے ڈبلیو ایف، اس کے ٹرسٹیز، بورڈ ممبران، عہدیداران، ملازمین، رضاکاروں اور نمائندوں کو میرے غیر مجاز اقدامات یا کوتاہیوں سے براہ راست یا بالواسطہ پیدا ہونے والے کسی بھی دعوے، نقصان، ذمہ داری، جرمانے، قانونی کارروائی، لاگت یا اخراجات سے محفوظ رکھوں گا/رکھوں گی، ان کا دفاع کروں گا/کروں گی اور انہیں بے ضرر رکھوں گا/رکھوں گی۔
10. I acknowledge that this Undertaking shall remain binding throughout my membership and shall survive the termination, suspension, resignation, or revocation of my membership with respect to any acts committed during my association with JWF.
میں تسلیم کرتا/کرتی ہوں کہ یہ یقین دہانی میری رکنیت کے دوران پابند رہے گی اور میری رکنیت کی معطلی، خاتمے، استعفیٰ یا منسوخی کے بعد بھی اُن اقدامات کے حوالے سے نافذ العمل رہے گی جو میری وابستگی کے دوران کیے گئے۔
11. This Undertaking shall be governed by and construed in accordance with the laws of the Islamic Republic of Pakistan, and the courts having jurisdiction in Pakistan shall have exclusive jurisdiction over any dispute arising hereunder.
یہ یقین دہانی اسلامی جمہوریہ پاکستان کے قوانین کے تحت چلائی اور ان کے مطابق تشریح کی جائے گی، اور اس سے پیدا ہونے والے کسی بھی تنازعے پر پاکستان میں دائرہ اختیار رکھنے والی عدالتوں کو خصوصی دائرہ اختیار حاصل ہوگا۔
